A: A Home Medicines Review (HMR) is a service provided by an accredited pharmacist after they receive a referral from your doctor. The pharmacist will make an appointment with you to come to your home and help you with your medication.
Q: Is there any charge for a Home Medicines Review?
A: No, this service is free of charge and is funded by the Australian Government.
Q: What is done in a Home Medicines Review?
A: The pharmacist will go through all of your medication to make sure you understand everything you are taking. They will answer any questions you may have and help to make sure you are taking your medication so that it can help you as much as possible.
Q: Does the Pharmacist need to be specially qualified to conduct the medication review, or can all pharmacists conduct them?
A: In order to conduct a Home Medicines Review the pharmacist must have done extra training and be accreditation. These pharmacists are called "Credentialed Pharmacists".
Q: I think a medication review would benefit me, what should I do?
A: Discuss it with your doctor and if you both agree, the doctor can do a referral

A: Yes, a home medicines review can be very helpful both to increase your understanding your medications, and for us to help you with strategies and options to help you to manage and take your medications as per your doctors directions.
Q: Is my privacy guaranteed?
A: Yes, we will not discuss any information that we receive or learn from conducting the home medicine review with anyone that you have not agreed to. The current consent form has the information for you. Once you have read it, please contact us or ask at your review if you have any questions. Your privacy is very important to us and is just as important as helping you with your medication and health.
Q: Does the pharmacist and/or doctor provide a follow-up from my home medicines review?
A: Yes, your pharmacist sends a report to the referring doctor, who will discuss it with you. If it is necessary, the accredited pharmacist may also schedule follow-up appointments with you to check on your progress.
Q: My doctor is unsure how to do a referral, what should I do?
A: You can either ask your doctor to contact us at ramseykr@protonmail.com , refer the doctor to this website, or print out this referral form and give it to your doctor.
