Q: Do you offer Reviews for Residential Aged Care residents (RMMR - MBS item 903) and what are they?
A: Yes, we do offer RMMRs. A RMMR Service consists of a Patient Interview, clinical assessment and written initial RMMR report provided to the Referrer and the multidisciplinary team and up to two follow-up services if clinically indicated.
Q: Does the patient or facility have to pay for a RMMR? and does the doctor get paid via the MBS?
A: No and Yes. This service is free of charge to the patient and facility through referral from their doctor/appropriate specialist. General Practitioners are entitled to a MBS fee (Item 903) of

Q: I am a doctor who provides services to a Residential Aged Care Facility, where do I find more information about RMMRs?

Q: Does the nursing home require a contract with the accredited pharmacist to perform RMMRs? and can we get you to do a review if we already have another contracted provider?
A: Yes the nursing home is required to have a RMMR service agreement before RMMRs can be performed. From 21/4/2020, a nursing home may enter into agreements with more than one provider and the requirement that only one service provider may be contracted with a single facility is removed. Q: Does the Pharmacist need to be specially qualified to conduct the medication review (DMMR/HMR/RMMR), or can all pharmacists conduct them?
A: In order to conduct a Home Medicines Review or a Residential Medication Management Review, the pharmacist must have done extra training and accreditation. These pharmacists are called "Credentialed Pharmacists".
Q: I am a doctor, how do I do a referral for a RMMR?
A: Many of the current prescribing software can create a referral for you, otherwise if we have a service agreement with your RACF, we can send you a referral form. Please ask your RACF if you would like us to conduct reviews for your residents.
Q: How often can I get a RMMR done for an eligible resident?
A: A subsequent RMR may only be conducted if more than 24 months has elapsed since the date of the most recent Patient Interview or when the referring doctor specifically deems a subsequent review is clinically necessary, such as when there has been a significant change to the Patient’s condition or medication regimen.
Reasons why an additional review may be requested include but are not limited to:
•Discharge from hospital after an unplanned admission in the previous four weeks
•Significant change to a patient’s medication regimen in the past three months
•Change in medical condition or abilities (including falls, cognition, physical function)
•Prescription of a medicine with a narrow therapeutic index or requiring therapeutic monitoring
•Presentation of symptoms suggestive of an adverse drug reaction
•Sub-therapeutic response to therapy
•Suspected non-compliance or problems with managing medication-related devices

A: Yes, our operations are governed by strict privacy principles. We must obtain consent prior to conducting the Patient review to allow sharing of necessary Patient information between health care providers.
